How many GP's who have retired but continued to practice, since
2004, have had their entry on the performers list removed and had
to reapply?

Many thanks for your Freedom of Information Request (FOIR 272) and
interest in Salford PCT.
Following enquires we can clarify the following:-
No GP's who have retired since 2004 have continued to practice, have had
their entry on the performers list removed and had to reapply
Since 1 Jan 2004 to 30 June 2008 16 GP's will have retired. Of these
4 of them are still on the PMP List
8 stated that they did not want to stay on the PMP List
3 have resigned from the PMP List since their initial retirement from GP
1 moved to another PCT PMP List
